Relapse Prevention

 
 
Assess need for psychological intervention
Assess need for pharmacological intervention and have full discussion about preferences of service user
Discuss with service user continuing antipsychotic for 1 - 2 years
 
 
 
   

Especially if:

  • At risk of relapse.
  • Recent Relapse.
  • Persistent symptoms

Especially if:

  • Persistent symptoms.
  • Lack of insight.
  • Poor treatment adherence
Oral Antipsychotic

Depots if:

  • Service user prefers, or
  • Avoidance of covert non-adherence is a clinical priority
 
 
     
Family Intervention
CBT
  • Continue dosage regimens
  • Standard doses
  • One antipsychotic only
  • Use test dose according to BNF
  • Use within standard dosage and interval ranges
 
         
More than 6 month course of treatment with more than 10 sessions

Consider service user preference for:

  • Mode of administration
  • Organisation of administration (where and from whom)

 

Monitor Clinical Response and side effects

Clinical Response
  • ESP/akathisia,
  • Weight gain/diabetes,
  • Sexual Dysfunction,
  • Lethargy,
  • Effects on eyes
Assess for work, education and other occupational Activities
Help individuals find employment and/or physical and social activities

 

Side Effects
Consider community resources for service users and carers
If individual remains ill or has persisting symptoms
Consider treatment resistance
