The Trust is part of our Community. We value the knowledge and experiences of services users, carers and our communities. We are committed to using them in all our decision-making processes. We promise to ask for the views of the people we serve and to be responsive to suggestions on how we can improve. We are particularly proud of the work our staff do. We work closely with our partners to develop and deliver services that meet the community's needs. In a World of change we will continue to support them in maintaining the highest standards of personal performance and professionalism. The Trust tries to serve the community in the best way it possibly can. I hope that you will find this website useful. |

This service is specifically designed to ensure the safety of Health Care Professionals within the 5 Boroughs who work out of hours (9-5). In order to register yourself as a lone worker you need to provide the following information:
If a registered Lone Worker does not contact the Call Centre by an agreed time the Call Centre Operator(s) will immediately engage in the task of attempting to contact the worker to Assess their safety. If necessary we will contact the relevant team manager and/or the Emergency Services. In order to use this service please call: 0800-0834323
Any care professional from the 5 Boroughs Partnership NHS Trust who requires access to E.C.P.A (Essential Care Programme Approach) can contact the call centre to register their Username and Caller Identifier.
The database holds confidential client information for known service users across the Trust. As well as detailing the basic demographic details of clients. All records have detailed risk assessments, relapse signatures and contingency plans for each client.
Our service offers 24 hour access to often vital patient information that is required at times of crisis or sudden relapse.
Service users, carers and the public can contact the call centre for advice about services across the 5 Boroughs on the 0800-0834323 (freephone number).
The Call Centre staff will listen to the Caller's needs and inform them which service they need as well as advice on how to access this service.
All calls are treated in the strictest of confidence
We hold on file details for all Mental Health, Learning Disabilities and Drugs & Alcohol Services within the 5 Boroughs. We also have details for agencies and services outside of the Trust, and for National Organisations and Charities.
