Effective Care Co-Ordination is applicable to all adults of working age in contact with the Secondary Mental Health System, which incorporates both health and social care. The same principles are to be applied to other care groups the Trust serves, including older people's services, learning disabilities, substance misuse, primary care and child and adolescent services. Effective Care Co-ordination supports good professional practice and includes all staff employed by the trust. |
