Description of the Service

Single point of referral to all parts of the team ie Day Services, Community and Cognitive Clinic, for people over the age of 65 with functional/organic problems

 Eligibility Criteria

Referral by GP or other hospital services for men and women over the age of 65 (and under 65 with memory problems) presenting with moderate to severe mental health problems ie functional/organic

Referral Forms and Route of Referral into the Service

Via referral form and also GP letter

Treatment Options – What is Accessible Through the Team?

• Assessment, using Effective Care Co-ordination (ECC) by Community Psychiatric Nurse or Occupational Therapist.
• Monitoring mental health and effects of medication.
• Administration of depot medication.
• Providing support to client and carers.
• Home assessment using PEO model

Recovery – How Does the Team Support Recovery?

• Individual care planning and reviews using ECC.
• Interagency liaison and requests for further assessment.
• Multi Disciplinary Team meeting to agree discharge planning

• Verbally
• Compliment letters and cards
• Quality Questionnaire currently being developed
