Knowsley South Community Mental Health TeamAddress: Car Parking is available. There are 20 parking bays around the building. Central Knowsley Community Mental Health Team (CMHT) is part of the Five Boroughs Partnership NHS Trust and Knowsley Borough Council, Social Services Department. The CMHT provides a service for people experiencing a moderate to severe mental health problem who live within Huyton, Huyton with Roby, Stockbridge Village and Knowsley Village. The CMHT works with individuals, carers, families, neighbourhoods and other agencies. The CMHT provides a point of contact and a route to specialist mental health services Opening HoursThe CMHT currently provides a service between 9am to 5pm, Monday to Friday. The service is aiming towards a more flexible approach and may extend to being available from 8am to 8pm. in the near future. Some routine CMHT work is carried out at weekends by arrangement. Team Composition:The CMHT combines the resources of social and health care services. Therefore, the team is staffed by community psychiatric nurses, approved social workers, occupational therapists, nurse therapists, psychology, community mental health workers/community support workers, consultant psychiatrists and admin workers. The team is source of skills and knowledge in the area of mental health. The CMHT coordinates support and care to services users via effective care coordination which combines the Care Programme Approach and care management. Referral CriteriaThe CMHT operates an open referral system. Initial screening and assessment is undertaken by a screening and assessment team and appropriate referrals are then forwarded to the CMHT. Treatment options Psycho-social interventions, Cognitive Behavioural Therapy, Anxiety and Depression Management, person centred counselling. How does the CMHT Support RecoveryThe CMHT supports recovery via aftercare services for patients discharged from hospital after an episode of illness. Recovery is also supported in the community for individuals who are not admitted to hospital during an episode of illness. Wherever possible, treatment and recovery in the community are supported in partnership with service users, particularly if an individual does not want to go into hospital. The CMHT links into the Home Treatment/crisis Team to support recovery. Disability Access and FacilitiesCar Parking - There are 20 parking bays around the building. There are no specific spaces for blue badge holders. Access – There are two steps at the front entrance of the building. A temporary ramp is available upon request. The rear of the building is level access. The two-storey building does not have a lift. All facilities for service users are on the ground floor. Induction Loop – The centre does not have an induction loop facility. Information & Correspondence – Information is not available in Braille, Large Print, Pictures/symbols or Moon. It is also unavailable on floppy disc or audio cassette. Communication – A Minicom system is not available. The centre does have access to a fax machine (No 0151 430 1841), but does not have an e-mail address. A sign language interpreter can be arranged upon request. Toilets – There is one unisex accessible (disabled) toilet on the main corridor, near the entrance. The Kirkby CMHT provides a service for people who are experiencing severe and enduring mental health problems or people who are under primary care but unable to receive an appropriate service. It also works with families carers and other agencies to help form a rounded service for the community in Kirkby. We assess and maintain people who suffer with a moderate to severe mental illness. Who is it for? Adults aged 16 – 64 years of age who suffer from a moderate to severe mental health problem. Who will you see? Clients may see any team member for an initial assessment, depending on the clients needs they may pass them over to a more appropriate team member to help with their care. Who else helps? - 24 Hr Access Team based at Whiston Hospital Where and when is this service available? The CMHT (Community Mental Health Team) operates 8.45am – 5.00pm Monday - Friday What do I do next if I think this service may help?
