Key Services
To provide a service which engages people with severe and enduring mental health problems who are otherwise difficult to engage and are at a higher risk of relapse. Provides a range of creative interventions which promotes social inclusion. Additional support to maintain people living in their own communities and provide an alternative to hospital admission.

9am - 5pm 7 Days per week

People experiencing severe and enduring mental health problems. We also provide advice and support to carers.

Criteria For Assertive Outreach

Clients aged between 18 – 65 who:

Must have complex and multiple needs because of severe and enduring mental health problems. Also need to include one of the following:

  • History of violence or persistent offending
  • Significant risk of persistent self harm or neglect
  • Dual diagnosis (substance misuse)

Detained under the Mental Health Act (1983) on at least one occasion on the last 2 years

Must have significant disability through their mental health problems and may require intensive periods of support

Must have experienced difficulty on engaging with traditional psychiatric services

Must be registered on Enhanced CPA and be accompanied by a thorough assessment to indicate this level of CPA

May be in contact with one or more services

May have experienced frequent hospital admissions (more than 2 admissions or more than 6 months in patient admission in the past 2 years)
